Claxton-Hepburn Medical Center’s Richard E. Winter Cancer Center now offers a Survivorship Care Program (SCP). The detailed and personalized SCP is designed to provide continued quality care to our patients who have been diagnosed with cancer and have graduated from their initial treatment plan. The SCP incorporates both physical and psychological well-being (cancer and non-cancer related) and provides patients with continuous access to our resources which may include continued physical examinations as needed, cancer screenings and prevention services, health and fitness tips, psychosocial well-being, and constant access to programs, education, and suggestions.

Together with our oncologists, patients will decide on an appropriate time to transition to the Survivorship Care Program. Once in the SCP, care will transition to a mid-level care provider at the Winter Cancer Center. The mid-level will continue to assess and monitor for any changes or concerns that may arise. Patients will also be provided with a personalized survivorship care plan, including health history, education, local resources, education, and tips relating to a person’s health and well-being.
