Position:     Social Worker/Social Worker Assistant

Status:        Part-Time 

Claxton-Hepburn Medical Center has an immediate opening for a Social Worker or Social Worker Assistant to provide discharge planning services to patients and their families who require assistance with discharge to home or alternate levels of care such as rehabilitation, nursing homes, physical therapy etc.  

Position Description:
Applies screening criteria, conducts psychosocial assessment, develops and implements  appropriate admission/discharge plans to meet hospital care goals and patient needs.  

Provides supportive counseling. Uses social work principles to accommodate patient and family  cultural, religious, and gender-specific needs.  Serves as patient advocate and assists medical personnel and families to understand social and emotional issues, patient needs and the admission/discharge plan.

Qualifications:
Masters Degree in Social Work preferred, Bachelor’s in social work or related field required. Must have experience and skills in human services, social work, or case coordination/discharge planning.
